• Reply to the automated appointment confirmation

• Arrive on time for your appointment

• Bring the completed "Check In" form if you haven't been here in the last year.

• New adult patients-please bring your current prescription for glasses and/or contacts.

• All patients please bring your current insurance card.

• Plan your time appropriately.  Many patients require dilation for their exam.  This adds time to the appointment prior to being seen by the doctor.  While you can resume normal activities 30 minutes or so following your appointment, we suggest not planning activities requiring acute vision such as competitive sports for the duration of the day.

• Diabetic patients' special care

  • Please let us know your condition upon check in.  Stronger dilation drops are needed.

  • ​Bring a designated driver or allow 30 minutes for the dilation to wear off.
